Form · FORM-30116

The Contractor Permit to Work is designed to ensure that contractors and safety officers adhere to critical safety protocols. It helps you manage contractors on site, so all safety checks and sign-offs are completed.
It records the contractor's details, the work description, start and finish dates and the location of the work, the high-risk construction work involved, such as confined spaces or asbestos disturbance, the SWMS, risk assessments and permits in place, and the insurance policies held.
The contractor signs a safety statement declaring their understanding of the relevant safety obligations, and the on-site manager signs off that the permits are issued and the site induction arranged. It is part of the Health and Safety Manual, whose documents refer to each other.
Suitable for construction companies, safety officers, project managers and contractors working in high-risk environments.
