Also known as: Scaffold collapse, Ladder failure, Access equipment failure
How to control scaffold and ladder failure
- Use licensed scaffolders where required, and confirm scaffolds are inspected and handed over as safe before first use.
- Inspect scaffolds at regular intervals and after storms, repairs or alterations, and keep a record of each inspection.
- Erect scaffolds and ladders on firm, level ground using base plates, sole boards or levelling feet.
- Keep loads within the scaffold's duty rating and the ladder's load rating, and never overload platforms.
- Use industrial-rated ladders, secure them at the top or base, and set extension ladders at the correct angle.
- Tag out damaged ladders and scaffold parts, and never remove ties, braces or guardrails without authorisation.
Typical controls, most effective first. Your SWMS sets the controls for your own job and site.
