Serious injury or fatality

The worst outcome of an uncontrolled hazard, such as a crush, collision or roll-over, where a person is killed or seriously injured.

Also known as: Fatality, Fatalities, Death

How to control serious injury or fatality

  1. Identify the tasks most likely to cause death or serious injury, and apply the highest-order controls to them first.
  2. Verify critical controls, such as exclusion zones, isolation and fall protection, are in place before high-risk work starts.
  3. Prepare a SWMS for high-risk construction work, and stop work if it is not being followed.
  4. Plan the emergency response for the worst case, including rescue, first aid and ambulance access to the work area.
  5. After a death or serious injury, preserve the scene and notify the WHS regulator immediately, as notifiable incident rules require.

Typical controls, most effective first. Your SWMS sets the controls for your own job and site.
