Corrective and Preventative Actions Form

A form to record a corrective or preventative action: the date, the details of the issue with any evidence attached, and how serious it is (critical, major or minor).

It records comments and immediate actions, the results of the evaluation and any cause found, how likely the issue is to recur, the action taken, the person or people responsible, the agreed completion date and the follow-up verification, and a manager or delegate signs off that the actions were effective.

It is part of the Health and Safety Manual, whose documents refer to each other.
