Formal Issue Resolution Procedure

The Formal Issue Resolution Procedure sets out how a worker raises a WHS hazard or concern: first with their supervisor or manager, who investigates it, acts on it, tells the worker the outcome and records it on a Hazard Report Form.

It says what happens when there is a severe immediate risk to workers, and its flow chart takes an issue that is not resolved through the health and safety representative (HSR), a provisional improvement notice (PIN) and the regulator's inspector.

It is part of the Health and Safety Manual, whose documents refer to each other.
