Risk Assessment Form

The Risk Assessment Form helps you weigh up how an item or service you are considering purchasing could affect the health and safety of your workers, contractors or others.

Its guidance sheet takes you through each step: the product's or service's hazards, its use and operation, worker competency, the work environment and the supplier's safety record, then rating likelihood and consequence on the risk matrix and writing down mitigation strategies.

The worksheet records each item or service, its potential risks, risk level, mitigation strategies and responsible person, with the assessor's name, job title and the date. Worked examples (a new forklift model and a welding contractor) and the hierarchy of control are included.
