Lock Out Tag Out - LOTO

SOP · SOP-60221

  • Version 4

The Lock Out Tag Out (LOTO) safe operating procedure helps a business put safe work processes in place when machinery or equipment is locked out and tagged for maintenance or servicing.

It sets out the steps to isolate every energy source before work begins and to restore the equipment to service afterwards, then a sheet for each machine: the PPE it needs, its hazards, the precautions and isolation devices, and its own step-by-step lockout.

A risk assessment guide, with the hierarchy of control and a risk matrix, and a risk assessment worksheet complete it. It is also part of the Health and Safety Manual system.
